The Motor Controller appears to be made by DART.
NEMA 4/12 Housing.
100V DC Shunt Wound Volts for Field.
0 to 90V DC Voltage Output to Armature.
1/4 to 1 hp Motors.
10A Max.
Fully Tested.
Warranted for 60 days against failure.
Just received this and several other DC Motor Controllers from a Company I do business with.  They are converting some of their Manufacturing Equipment to Variable Frequency Drives and have taken these out out service.
NOTE:  In order to be fully NEMA 4/12 compliant, the ON/OFF switch will need a new "Toggle Switch Boot".  Grainger should carry a replacement should you be so inclined.
